Background:Decision-making and assessment in emergency situations are complex and result many times in ethical conflicts between different healthcare professionals.Aim:To analyse and describe situations that can generate ethical conflict among nurses working in emergency situations.Methods:Qualitative analysis. A total of 16 emergency nurses took part in interviews and a focus group.Ethical considerations:Organisational approval by the University Hospital, and informed consent and confidentiality were ensured before conducting the research.Result/conclusion:Two categories emerged: one in ‘ethical issues’ and one in ‘emotions and feelings in caring’. The (...) four ethical subcategories are presented: Autonomy, the first sub category: first, the nurse’s ability to practise care on an emergency ward and, second, to support the patient and/or relatives in terms of care and medical treatment. The conflicts arise when the nurse ends up in the middle between the patient and the physician responsible for the diagnosis and treatment from a nature scientific perspective. Reification of injured body: patient was often reified and fragmented, becoming just a leg or arm. Different factors contributed in this perspective. Pain: pain relief was often inadequate but more effectively treated in the emergency medical services than at the emergency department. The nurses highlighted the phenomenon of suffering because they felt that pain was only an object, forgetting the patients’ care need, like separating mind from body. Death: the nurses felt that the emergency services are only prepared to save lives and not to take care of the needs of patients with ‘end-of-life’ care. Another issue was the lack of ethical guidelines during a cardiac arrest. Resuscitation often continues without asking about the patient’s ‘previous wishes’ in terms of resuscitation or not. In these situations, the nurses describe an ethical conflict with the physician in performing their role as the patient’s advocate. The nurses express feelings of distress, suffering, anger and helplessness. (shrink)

This paper tests whether individual perceptions of markets as good or bad for a public good is correlated with the propensity to report gaps in willingness to pay and willingness to accept revealed within an incentive compatible mechanism. Identifying people based on a notion of market affinity, we find a substantial part of the gap can be explained by controlling for some variables that were not controlled for before. This result suggests the valuation gap for public goods can be reduced (...) through well-defined variables. (shrink)

AimsA negative association between the lower level of psychological resilience and increased risk of compassion fatigue and higher Coronavirus disease 2019 stress has been revealed. However, bibliometric studies have not been performed to comprehensively investigate this topic. This study aimed to identify the status and trends in the CF and PR field from 2008 to 2021 and during the COVID-19 pandemic.MethodsWe identified relevant literature from the Web of Science Core Collection® database using “resilience” and “compassion fatigue” on September 30, 2021. All search results were exported in plain text format for collaboration network analysis, reference-based co-citation analysis, analysis of journals, and keywords-based co-occurrence analysis, which were performed using Citespace® 5.8.R1.ResultsA total of 388 publications were identified finally, and there has been an increasing trend in the annual number of publications with light fluctuations. The analysis of journals and keywords indicated that nurses and social workers are the main research targets, and their mental problems are the main research topics. The turnover intention of health care providers has been a research focus, particularly during the COVID-19.ConclusionThe results of the present study help us understand the status of the CF and PR field and its recent developments. ObjectiveThe impact of the mindfulness-based stress reduction program on psychological outcomes and quality of life in lung cancer patients remains unclear. This meta-analysis aimed to evaluate the effectiveness of the MBSR program on psychological states and QoL in lung cancer patients.MethodsEligible studies published before November 2021 were systematically searched from PubMed, EMBASE, Cochrane Library, PsycINFO, China National Knowledge Infrastructure, and Wanfang databases. The risk of bias in eligible studies was assessed using the Cochrane tool. Psychological variables and QoL were evaluated (...) as outcomes. We used the Grading of Recommendations Assessment, Development and Evaluation system to grade the levels of evidence. Statistical analysis was conducted using RevMan 5.4 and STATA 14.0.ResultsA total of 17 studies involving 1,680 patients were included for meta-analysis eventually. MBSR program significantly relieved cancer-related fatigue and negative psychological states, enhanced positive psychological states, and improved quality of sleep. Evidence on MBSR programs' overall treatment effect for QoL revealed a trend toward statistical significance.ConclusionBased on our findings, the MBSR program shows positive effects on psychological states in lung cancer patients. PurposeThe aims of the study were first to investigate the association between illness perception and psychological distress and second to determine whether mindfulness affects psychological distress via illness perception and perceived stress in patients with lung cancer.MethodsAmong 300 patients with lung cancer who participated in this cross-sectional study, 295 patients made valid responses to distress thermometer, the Five Facet Mindfulness Questionnaire, the Brief Illness Perception Questionnaire, and the Perceived Stress Scale between January and July 2021. The possible pathways of mindfulness (...) affecting psychological distress were analyzed based on the structural equation modeling analysis.ResultsA total of 24.4% patients with lung cancer had DT > 4. Illness perception and perceived stress had a direct effect on psychological distress. Mindfulness had a direct effect on illness perception and mindfulness indirectly influenced psychological distress through affecting illness perception alone or simultaneously affecting both the illness perception and perceived stress in patients with lung cancer.ConclusionLung cancer suffered from varying levels of psychological distress. Mindfulness may alleviate psychological distress by reducing the level of illness perception and perceived stress. We suggest developing a comprehensive factor model to clarify potential mechanisms of mindfulness on psychological distress due to the very low effect of mindfulness on psychological distress via illness perception and perceived stress. Background Healthcare professionals, especially professional nurses, experience various types of moral suffering due to inevitable ethical conflicts. Moral resilience is recently proposed as a resource to address moral suffering. However, there is no tool to measure moral resilience in Chinese professional nurses. Aim This study aimed to translate the Rushton Moral Resilience Scale (RMRS) into Chinese and evaluate the psychometric properties of the Chinese version of RMRS (Chi-RMRS). Research design A methodological and descriptive research design. Participants and research context A (...) convenience sample of 411 Chinese professional nurses was recruited through an online survey platform between February and March 2023. Ethical considerations This study was approved by the Research Ethics Committees of the University and hospitals involved. Results The RMRS was translated and culturally adapted into a Chinese version. Neither floor nor ceiling effects were observed. The scale-level content validity index (CVI) was 0.922 with the item-level CVIs ranging from 0.833 to 1.000. The explanatory factor analysis (EFA) generated a three-factor structure for the Chi-RMRS, and the confirmatory factor analysis (CFA) demonstrated the three-factor structure with factor loadings for each item ranging from 0.42 to 0.80. The scale-level Cronbach’s α coefficient was 0.811 with each dimension ranging from 0.717 to 0.821, and composite reliability (CR) coefficient for the overall scale was 0.920, with each dimension varying from 0.739 to 0.824. The standard error of measurement (SEM) and smallest detectable change (SDC) were 3.522 and 9.763, respectively. Discussion The Chi-RMRS is able to measure moral resilience of Chinese professional nurses, and has good validity and reliability. It can be used in research and practice to determine the level of moral resilience, thus helping nursing managers to monitor the status of Chinese professional nurses, then develop interventions to maintain the well-being of professional nurses and to ensure quality of care. BackgroundCommunity Advisory Boards (CAB) have become essential organs of involving communities in HIV clinical trials especially in developing countries. However, limited empirical evidence exists on the role of CABs in low and middle-income countries including Tanzania. This study aims at exploring the role of CABs in community-based HIV clinical trials conducted in Tanzania.MethodologyWe adopted a phenomenological approach to purposefully select HIV clinical trial stakeholders. These included CAB members, researchers and Institutional Review Board (IRB) members in Tanzania. We conducted In-depth Interviews (...) (IDIs) with ten participants and three Focus Group Discussions (FGDs) with eighteen participants. The data were thematically analyzed with the aid of MAXQDA software version 20.2.1.ResultsThe findings indicate that at every stage of implementation of a community-based HIV clinical trial, a functioning CAB is important for its success. This importance is based on contextualization of the informed consent process and protocol, managing rumours in the community, weighing trial risks and benefits, sensitizing the community, assisting participant recruitment, tracing and retention. However, being perceived as financial beneficiaries than community representatives emerged as a challenge to CAB members. ConclusionThe study empirically indicates the need for functioning CABs in every stage of implementation of community-based HIV clinical trials. The roles of which are interwoven in serving research goals and protecting the interests of the community and that of trial participants. (shrink)

RESUMEN La problemática del muy bajo peso al nacer ha sido abordada desde hace varios años, sin embargo, las investigaciones están enfocadas desde diferentes perspectivas y contextos, no siempre se ha considerado su repercusión a corto y a largo plazo, así como la interacción de los diferentes factores que se relacionan con este riesgo biológico. Por ello en esta revisión bibliográfica se realiza una valoración de estudios ejecutados a nivel internacional y en Cuba dedicados al desarrollo neuropsicológico de niños con (...) muy bajo peso al nacer. Se concluye que el muy bajo peso al nacer repercute en el desarrollo neuropsicológicos desde edades tempranas, hasta la adolescencia; por ello es necesario una visión integradora que tenga en cuenta el riesgo biológico, las particularidades individuales y el contexto social en que se desenvuelve el niño desde los primeros momentos, lo que posibilitará, perfeccionar la atención a corto y largo plazo al grupo de riesgo. ABSTRACT The problems of very low birth weight have been dealt with for many years, however, research on the matter is approached from different perspectives and contexts, not always have its short and long term repercussions being considered as well as the interaction of different factors related to this biological risk. Therefore, this bibliographical review makes an assessment of international and Cuban studies devoted to very low birth weight children’s neuropsychological development. It was reached the conclusion that very low birth weight affects neuropsychological development from early ages to adolescence; for that reason, it is necessary to have an integrated approach of the problem that takes into account the biologicals risk, individual characteristics and the social context in which the child develops from the beginning, which will make the improvement of short and long term care to this risk group possible. BackgroundThe safety of sleeping pills has increased dramatically during the last 100 years, from barbiturates to bensodiazepines to modern day so-called Z-drugs.MethodsThe circumstances of prescribing sleeping pills in the early 20th century are illustrated by summarizing the main storyline of a novel by Swedish writer Vilhelm Moberg. This is followed by a thought experiment and a theoretical discussion.ResultsIn his 1937 novel Sömnlös (Swedish for sleepless) Vilhelm Moberg portrayed existential and relational distress in relation to insomnia. In a thought experiment, past (...) progresses in sleeping pills safety are projected into the future. Thereby, it is claimed that important issues in the area of philosophy of medicine come to the fore. This leads to a theoretical discussion about broader questions concerning the role of the physician, the goals of medicine (as described by Lennart Nordenfelt), the concept of pharmaceuticalisation (as described notably by sociologist of sleep Simon J. Williams and co-workers), and health enhancement (c.f. Carl Elliott and the alleged wish to be better than well).ConclusionInsomnia is a prism through which important philosophical and sociological questions related to the practice of medicine can be asked. Background Speech understanding may rely not only on auditory, but also on visual information. Non-invasive functional neuroimaging techniques can expose the neural processes underlying the integration of multisensory processes required for speech understanding in humans. Nevertheless, noise (from fMRI) limits the usefulness in auditory experiments, and electromagnetic artefacts caused by electronic implants worn by subjects can severely distort the scans (EEG, fMRI). Therefore, we assessed audio-visual activation of temporal cortex with a silent, optical neuroimaging technique: functional near-infrared spectroscopy (fNIRS). Methods (...) We studied temporal cortical activation as represented by concentration changes of oxy- and deoxy-hemoglobin in four, easy-to-apply fNIRS optical channels of 33 normal-hearing adult subjects and 5 post-lingually deaf cochlear implant (CI) users in response to supra-threshold unisensory auditory and visual, as well as to congruent auditory-visual speech stimuli. Results Activation effects were not visible from single fNIRS channels. However, by discounting physiological noise through reference channel subtraction, auditory, visual and audiovisual speech stimuli evoked concentration changes for all sensory modalities in both cohorts (p<0.001). Auditory stimulation evoked larger concentration changes than visual stimuli (p<0.001). A saturation effect was observed for the audiovisual condition. Conclusions Physiological, systemic noise can be removed from fNIRS signals by reference channel subtraction. The observed multisensory enhancement of an auditory cortical channel can be plausibly described by a simple addition of the auditory and visual signals with saturation. (shrink)

L’article montre d’abord jusqu’où convergent la dialectique hégélienne de l’Idée et la dialectique lautmanienne des Idées, et ce sur quoi elles se séparent en profondeur : sur la négativité et le statut de la contradiction. Il s’intéresse ensuite à certaines formalisations qui ont été proposées de ces deux dialectiques : celle de Doz et Dubarle pour Hegel dans une extension de la logique booléenne, et celle, récemment esquissée par F. Zalamea en théorie des catégories, pour Lautman. Est montré dans ses (...) grandes lignes comment la traductibilité mutuelle, au niveau technique, peut être établie entre les deux entreprises, la conséquence étant que la divergence spéculative semble rétroactivement gommée, que le négatif semble avoir disparu. À partir de ce paradoxe sont présentées quelques pistes de réflexion sur les enjeux de la démarche de formalisation, entendue comme entreprise de réduction du fossé existant entre le conceptuel et le formel, et de ce qu’elle révèle des rapports entre mathématiques et philosophie.The paper begins by showing what is common to Hegel’s dialectic of the Idea and Lautman’s dialectic of Ideas, and where they diverge deeply : on negativity and the status of contradiction. It then focuses on two different attempts to formalize these dialectics : Doz and Dubarle’s attempt to formalize the Hegelian one in an extension of Boolean logic ; and, more recently, Zalamea’s formalization of the Lautmanian one through category theory. The paper then sketches how, at a technical level, these two attempts can be translated one into the other — which indicates, in turn, that the speculative divergence seems to have disappeared, that the negative has been erased. With reference to this paradox the paper provides the reader with some reflections dealing with the stakes of the formalizing process, seen as way of reducing the gap existing between the conceptual and the definite, and of what it reveals of the relations between mathematics and philosophy. It relies essentially on the issue of the nature of knowledge and truth, and the way we may apprehend it, either immediately or mediately. Les normes au sens le plus large sont issues des interactions entre les individus et sont interprétées au sein de ces interactions, même si la complexité des interactions tient aussi à l'intervention des institutions et des Etats. Il s'agit de partir du micronormatif pour aller vers le macronormatif (notamment les lois et les traités) et non l'inverse, de manière à rendre compte de phénomènes comme la responsabilité sociale des entreprises, le droit souple et la prise en compte de l'éthique pour (...) prendre des décisions (au plan national et international, en matière environnementale et médicale, etc.). Le constat de départ est qu'une masse colossale de normes de toute origine s'applique à des individus, personnes physiques ou morales, qui à leur tour participent de l'élaboration de normes. L'inflation et la diversification des normes engendrent notamment une crise du droit (en quête d'identité), mais aussi une crise des institutions et des professions judiciaires. Une façon de rendre compte de ces crises consiste à établir une sorte de cartographie des interactionnismes au regard des normes. Emergent ainsi des interactionnismes philosophiques, sociologiques, anthropologiques, psychologiques, éthiques ou juridiques. Les différents interactionnismes peuvent apparaître critiquables au regard des normes (notamment en raison de leur caractère descriptif et non institutionnel) et il serait fort utile de dégager des voies alternatives. L'introduction met en place cette problématique (E Picavet et E Jeuland). Après une recherche théorique et pratique menée par des juristes (E Jeuland et L Champain), un sociologue des professions (F Champy) nous permet de penser l'évolution des professions judiciaires entre statut et adaptation. Une philosophe spécialisée dans l'interactionnisme (C Bonicco) ainsi qu'une psychanalyste (H Tessier) s'interrogent sur les normes implicites, non dites et les rapports de domination sous-jacents aux interactions. Un philosophe spécialisé dans la justice climatique et le nucléaire ouvre un nouveau champ de réflexion particulièrement nécessaire aujourd'hui en incluant la nature au sein des interactions humaines (F P Piguet). Deux ingénieurs nous présentent une approche des interactionnismes et de l'innovation technologique (J F Omhover et C Bouchard) qui n'est pas sans lien avec la nature et le droit. Un anthropologue estime que le droit tel qu'il est connu depuis deux ou trois siècles dans une version très étatique est en plein bouleversement et revient à des formes microjuridiques (Etienne le Roy). Un juriste essaie enfin de faire la synthèse des articulations entre les normes et les interactions (J Moret-Bailly). Il en ressort une présentation transdisciplinaire des différents versants du problème de la relation entre les normes et les individus. (shrink)

The health and human rights movement (HHR) shows obvious signs of maturation both internally and externally. Yet there are still many questions to be addressed. These issues include the movement’s epistemological status and its perspectives of development. This paper discusses critically the conditions of emergence of HHR, its identity, its dominant schools of thought, its epistemological postures and its methodological issues. Our analysis shows that: (a) the epistemological status of HHR is ambiguous; (b) its identity is uncertain in the absence (...) of a validated definition: is it an action movement, an interdisciplinary field, a domain, an approach, a setting or a scientific discipline? (c) its main schools of thoughts are defined as “advocacists”, “ethicists”, “interventionists”, “normativists”; (d) the movement is in the maturation process as a discipline in which “interface”, “distance”, “interference” and “fusion” epistemological postures represent the fundamental steps; (e) parent disciplines (health sciences and law) competences, logics and cultures introduce duality and difficulties in knowledge production, validation and diffusion; (f) there is need to re-write the history of the HHR movement by inscribing it not only into the humanitarian or public health perspectives but also into the evolution of sciences and its social, political and economical conditions of emergence. The ambiguous epistemological status of this field, the need to re-write its history, the methodological duality in its research, the question of the competence of the knowledge validation, as well as the impact of HHR practice on national and international health governance are the challenges of its future development. To meet those challenges; we call for the creation and implementation of an international research agenda, the exploration of new research topics and the evaluation of the movement’s contribution to the national and global public health and human rights governance. En plein accomplissement de l’idéalisme allemand, Baader trouve chez le « cordonnier de Görlitz » les munitions spéculatives nécessaires pour lui opposer un « réalisme transcendantal » puissant et pleinement chrétien. C’est dans la compréhension, ardue mais salutaire, du juste rapport de la manifestation de l’être à sa médiation intérieure, que Baader pense découvrir (...) l’apport le plus excellent de Boehme à une philosophie théologique. Este escrito intenta aproximarse a la experiencia del suicidio a partir de Emmanuel Lévinas y sus conceptos filosóficos de “el decir” y “lo dicho”, los cuales ayudarán a mostrar al otro, el suicida, como una víctima invisible. El suicida necesita no ser tratado como un ser excluido y que debe ser medido desde la objetividad. Él requiere de un soporte que lo ayude a vivir desde la reflexión filosófica. Por lo tanto, el suicida verá en la filosofía un lugar (...) para el tratamiento del alma. Para lograr esta aproximación, este escrito se divide en tres partes que siguen a una breve introducción: en primer lugar se abordan las relaciones entre suicidio, subjetividad e intersubjetividad indicando la importancia de clasificar el suicidio en el decir y no en lo dicho, y poniendo de presente la lucha del suicida para ser escuchado en el mundo del decir, de modo que prevalezca el cuidado del otro en la subjetividad e intersubjetividad; enseguida, a través de la exposición de las nociones de huella, herida y experiencia de extranjero se aborda la relación y el encuentro entre el decir y lo dicho; y, finalmente, se propone que la filosofía, practicada como un abrazo “entre otros” es una vía para procurar la comprensión y el acercamiento con el Otro. Unas cuantas –aunque quizá no suficientes- líneas dedicadas a la Erinnerung del Maestro de siempre, acompañadas de la exégesis de un texto de su autoría: El concepto de “Saber aparente” en Hegel o la ilusión del método. Se trata de un ensayo de Giulio F. Pagallo que, hasta el presente, no ha sido publicado, aun cuando sí presentado como papel de trabajo en el marco de un Seminario organizado por el Instituto de Filosofía de la UCV a finales de los (...) años ’70. Quizá sea éste uno de los ensayos más importantes de su interpretación de la filosofía hegeliana, particularmente de la Fenomenología del Espíritu, y de sus estrechas relaciones de fondo con el Tractatus de Intellectus emendationis de Spinoza. Todo ello, animado por la firme convicción de contribuir al debate contemporáneo con el estructuralismo marxista y con el neopositivismo, como respuesta frente a las malversaciones propias de un uso maniqueo del concepto de ideología. Amicus IuliusSome –but maybe not enough- lines devoted to de Erinnerung of the Master forever, together with the exegesis of a text of his authorship: The concept of “Apparent Knowledge” in Hegel or the illusion of the method . This is a Giulio F. Pagallo’s essay which has not been published until today, even though it was presented as paperwork in a seminar organized by the Institute of Philosophy of the UCV in the last years of the 70`s decade. Perhaps, this is one of the most important essays of his Hegelian philosophy interpretation, particularly of the Phenomenology of the Spirit , and its tight and deep relationship with the Spinoza’s Tractatus de Intellectus emendationis . All of this is animated by the firm conviction to contribute with the contemporary debate with Marxist structuralism and new positivism, as a response upon the manichaean use of the concept of ideology. RESUMEN Este artículo explora si hay algún fundamento sólido para atribuir a Leonardo da Vinci prioridad en la formulación del símil que lleva a concebir el ojo como una cámara obscura. Aquí, se defiende una posición pesimista. No obstante, el artículo resalta algunos aportes del pintor renacentista que pueden considerarse contribuciones a la consolidación del símil. ABSTRACT The article explores whether there is any solid basis for attributing to Leonardo da Vinci the priority in formulating the simile that leads to (...) conceiving the eye as a camera obscura. Here, a pessimistic position is defended, however, the article highlights some of Leonardo's contributions to the consolidation of the simile. (shrink)

In his 1961 monograph, Totality and Infinity: An Essay on Exteriority , the late phenomenologist, Emmanuel Levinas, noted that “everyone will readily agree that it is of the highest importance to know whether we are not duped by morality” (1961/1969, p. 21). What follows thereafter is an extensive attempt to ground a quasi-Kantian existential ethics based on interpersonal, face to face, relations (Beavers 2001). That philosophy should invite such an attempt already signifies that we might be in trouble where (...) ethics are concerned. Emmanuel Levinas is of Jewish origin; born in Lithuania, he has lived, for his Jewish condition, the horrors of Second World War. His ethical proposal finds roots in the Jewish atmosphere, what provides him a hard criticism to the western thought. According to him, instead of proposing an ethics, it proposed a philosophy of the power. For him the ethics precedes not only the Philosophy and also the Theology and the Human Rights, so that the implication of Levinas’ thought (...) to the religion and the Human Rights is to put the relationship, the meeting with the Other as previous to any theme to keep the ethics and to avoid summarizing the Other in the Same. Emmanuel Levinas’s philosophy demonstrates that intelligence is ultimately at the behest of responsibility. He is one of a number of philosophers who made the paradigm shift from an individualized notion of self to a social conception of self. He used the language of his teachers, Husserl and Heidegger, to move beyond their philosophies to a fundamental paradigm shift, in which ethics is prior to epistemology.
